Official GRE, GMAT or MCAT test score report

Official GRE or GMAT test score reports must be sent by ETS, directly to our office at the address noted above. Photocopies of test score reports or unofficial records will not be accepted.

In the case of MCAT reporting, the applicant must provide the paper report along with the verification code. We will contact the test administrator to verify your MCAT test scores.

Three letters of recommendation

While we do offer a paper process, we strongly encourage recommenders to complete the online process. Paper recommendation forms should be signed by the applicant and each letter of recommendation should be received in a sealed envelope with the recommender's signature across the seal.

Recommenders should know you well and be able to evaluate your academic or professional work, your abilities to undertake graduate study and your potential as a public health professional.

If you have been out of school for less than 3 years, you must provide at least one academic reference.

Official TOEFL test score report

Foreign nationals whose undergraduate, graduate or professional degrees were obtained from a country where English is not the primary language of instruction must submit an official score for the Test of English as a Foreign Language (TOEFL).

All other documentation should be submitted through the on-line application process, including the $60 application fee.
